Finasteride increases the amount of testosterone in the body, by inhibiting its conversion into DHT. And increased testosterone will increase acne.

But ever since I have been on propecia my hair has been the exact same. Just the crown got slightly worse.

This is correct. However, as I note elsewhere, because finasteride differentially protects hair follicles, finasteride-induced exacerbation of balding is unlikely in the absence of other hyperandrogenic sysmtoms. At least I hope so. OTOH, if (say) your acne flares on finasteride while your hair loss gets worse, you may want to reconsider treatment.
