Mr. Susmit Gogate visited Hairfree & Hairgrow Clinic with concerns about progressive hair loss and baldness. He had been experiencing hair fall for about 6–7 years and noticeable baldness for the past 2–3 years.

At the time of consultation, he presented with Grade V–VI baldness, including hairline recession and thinning in the front and mid-scalp regions.

Consultation and Diagnosis​

After minimal improvement from conservative treatments, Mr. Gogate consulted Dr. Jinkal Kunjadiya, dermatologist and hair transplant surgeon at the Hairfree & Hairgrow Clinic, Surat branch.

A detailed scalp analysis, including trichoscopy, was performed to assess both donor and recipient areas. His donor area showed thick, healthy, black hair, making him an ideal candidate for transplantation.

Step-by-Step Hair Transplant Procedure​

1. Hairline Design and Planning​

A personalized hairline design was created by Dr. Jinkal Kunjadiya according to the patient’s facial proportions and expectations. The frontal and temporal zones were marked to achieve a natural and age-appropriate appearance.

2. Pre-Procedure Preparation​

An allergy and sensitivity test was conducted for local anesthesia, and no allergic reaction was observed.
The scalp was shaved and cleaned with betadine solution before the surgery.

3. Local Anesthesia​

A modified block anesthesia technique with vibration-assisted pain control was used to ensure comfort during the procedure.

4. Slit Creation​

Using sapphire tip blades (size 1.2 mm for the hairline and 1.4 mm for other areas), micro slits were made under magnification. Proper angulation and direction were maintained to ensure natural growth patterns.

5. Graft Extraction​

Extraction was done using a 0.8 mm sharp, serrated titanium punch.
Extraction time: approximately 130 minutes.
Graft quality:
  • 20% single grafts
  • 80% multiple grafts (double/triple follicular units)
  • Average of 3 hairs per graft, ensuring high density
The extracted grafts were stored in Ringer’s lactate solution at 4°C using a graft chiller machine.

6. Graft Implantation​

Implantation was performed using K.E.E.P. Implanters of different sizes suitable for 1-, 2-, and 3-hair grafts. The process took approximately 2.5 hours.

After implantation, sterile dressing was applied, medications were prescribed, and detailed postoperative care instructions were given. The patient also received a direct follow-up contact for any queries.

Post-Transplant Journey of Mr. Susmit Gogate​

Shedding Phase (0–2 months)​

  • During the first two months, about 70% of implanted hairs shed, which is a normal part of the process.
  • The first PRP session was performed at two months.
  • Existing native hairs showed a good response to treatment, and hair fall reduced significantly.
  • The donor area healed completely within 30 days.

Baby Hair Growth Phase (3–5 months)​

  • At around three months, new hairs began sprouting from the scalp.
  • The second PRP session was performed to stimulate growth.
  • By the fifth month, noticeable improvement in frontal density was observed.

Active Growth Phase (6–9 months)​

  • Around the sixth month, approximately 90% of transplanted hairs had grown well.
  • Although the length varied, the density appeared natural.
  • A booster PRP session was performed at nine months.

Final Results (12 months)​

  • By twelve months, Mr. Gogate achieved full and natural-looking hair coverage.
  • He could style his hair freely and had undergone multiple haircuts for both donor and transplanted areas.
  • He continued with Minoxidil 5% for maintenance and was advised to get occasional booster PRP as needed.
