merck claims that 83% of the men in their study at least kept all the hair they had when they started (baseline). Now since it takes at least 3-4 months to start working, you will still lose hair in those months, thus taking you below baseline. so my question is does propecia really keep the hair you had at baseline, and I am assuming frontal hair was kept also, otherwise how could they claim the men lost no more hair?

Finasteride achieves near maximum 5ar inhibition within 24hrs. So, in other words, Finasteride works the first day you start taking it. However, results can take 3 - 6 months or more to see because hair only grows 1/2 an inch per month and any hair that is in telogen (resting phase when the hair sheds) can take 3 - 5 months to start growing again.

Note:

Finasteride reduces scalp DHT by about 60%. Furthermore, DHT is not the only androgen present in the hair follicles. Testosterone, for one, is also an androgen and is not reduced by Finasteride use (in fact, Finasteride may increase testosterone levels).

This being said, depending on how sensitive your hair is to the remaining androgens is what likely determines to what extent hairloss slows or halts (Note that frontal areas of your scalp tend to be the most sensitive to androgens and the less responsive to medications).
