Today we have a case study and review of our patient Jahir Raihan. We will go through his full hair transplant journey in this article.
Location: Originally from Dhaka, Bangladesh (currently living abroad)
Duration of Hair Loss:
Jahir has been suffering from Grade 5 to 6 baldness for the past 18 years. His hair loss began at the age of 20 and gradually worsened with significant recession of the hairline, frontal baldness, and crown thinning. By the time of consultation, Jahir had lost almost all frontal and mid-scalp hair, with some sparse density in the crown.
Other Problems:
Mild dandruff and oily scalp were present, especially under humid conditions. No major scalp infections or inflammatory issues observed.
Family History:
There is a strong genetic history of male pattern baldness on both sides of the family, especially among maternal uncles and fathers.
Medical History:
The patient is non-diabetic, has normal blood pressure, and has no chronic illness. Routine pre-surgical tests were all within normal range.
Surgical History:
No major past surgeries were reported.
Allergy History:
No known allergies to food or medications.
Keloid History:
Patient has no history of keloid formation or abnormal skin healing.
Past Treatment:
Jahir had tried minoxidil briefly in his late 20s but discontinued due to lack of results and travel lifestyle. No use of finasteride or other medical treatments. Occasional use of natural oils but without any consistent benefit.
A high-density frontal and mid-scalp hairline design was created by the expert team under Dr. Naznin Nipa . Priority was given to natural aesthetics and strategic use of multi-root grafts to improve coverage with limited donors.
2. Trichoscopy Analysis:
The donor area was found to be strong with thick, mildly wavy black hair. Hair caliber was excellent, and the extraction plan was mapped in zones.
3. Anesthesia Sensitivity Test:
The skin sensitivity test showed no allergic reaction to anesthesia.
4. Preparation:
The scalp was shaved and sterilized with betadine and alcohol before surgery began.
5. Anesthesia:
Painless local anesthesia was given using a vibration-assisted technique to reduce discomfort.
6. Extraction:
Using a 0.8 mm titanium punch, donor grafts were carefully extracted using a magnifying loupe. Total time: ~3 hours over both days.
Graft Breakdown:
Grafts were preserved in RL solution inside a graft chiller at 4°C to maintain viability and strength.
8. Slit Making:
Slits were created using a sapphire blade 1.2 mm for the hairline and 1.4 mm for the frontal and mid-scalp areas. Slit angles were carefully planned to mimic the natural direction of growth.
9. Implantation:
Using the K.E.E.P. implanter, grafts were placed according to follicle type and desired density.
Total implantation time across two days: ~5 hours
10. Post-Procedure:
Dressing applied. Post-op medications, scalp care routines, and lifestyle precautions were explained. Jahir was given a direct follow-up line for any questions or emergencies.
