What could that be besides male pattern baldness? I think its safe bet its male pattern baldness already advanced stage.

So is there no hope for me since I didn't catch it in early stages? Would cutting my very long hair help with growth? I wasn't aware male pattern baldness could be in its advanced stage in my early 20s. :/

There are people here that have much worse male pattern baldness that you and they are also in their early 20s. There is hope for you. The crown part is usually the easiest to regrow with a combination of Minoxidil, Finasteride and Ketoconazole shampoo.

If you use minoxidil cutting your hair would be best as it's easier to apply it directly to the scalp.

Are all of these necessary to use in combination? Also, what is the most generally favored Ketoconazole shampoo?

Finasteride to block DHT systemically + maintenance.
Ketoconazole to block DHT topically on your scalp (much less effective and powerful than Finasteride so it is not a substitute but rather a complement; but a very good one at that)
Minoxidil to promote regrowth of hair + maintenance.

Regenepure is a very popular ketoconazole shampoo because it's sulfate free.

I keep seeing finasteride promoted, but I also keep seeing very serious side effects like an increased likelihood in prostate cancer. I know it's a simple question to ask, but is it worth the risks? Do you or anyone else in this thread have experience with it?

I've read the side effects are not too common. I've been on Fiansteride for about 3 weeks now. Minor side effects have been watery semen and maybe feelings of being tired, but that could also be due to my erratic sleeping times.

Alright, recently cut my hair and am posting more pictures from the top/back. This also shows my hairline.

How bad does this look, for someone at 22 specifically? And what would be the best doctor to make an appointment with, a dermatologist?

pretty bad. I would just buzz it for now while you work on regrowing hair. It would look better than what you're working with.

If you're just looking to get prescribed finasteride, you can see your general doctor or a derm. If you're looking for anything more in-depth than that, I would make an appointment with a derm.

Definitely balding in the crown and receding in the front.

I'm not sure if I can do that, I have some scarring on my face that I really don't want to bring further attention to by shaving my head. It was hard enough to cut it short like this.

Are there any possible alternatives while I get treatment? And how long on average would it take for at least some improvement in hair regrowth? Sorry for the barrage of questions.

About 4 months before most people see improvement. You're definitely more than an NW2, probably NW3, very deep temple recession and forelock formation. If you're lucky, you'll get back into NW2 territory with finasteride and minoxidil. You'll probably thicken up and regrow a bit, maybe not as much as you'd like, but a significant improvement on the present.
