Balding crown - nothing seems to work

Need advice. I’m considering shaving my head and getting SMP for my balding crown. I feel like I’ve tried every single hair loss treatment option over the last 7 years to no avail. Minoxidil, Finasteride, Dutasteride, Nizoral, Biotin, Microneedling. I actually think since adding dutasteride, my hair has gotten slightly worse.

My current routine includes topical minoxidil 1x day, Topical Dutasteride with Tretinoin & Biotin 1x day, and microneedling 1x month

is there anything else I should try? Is my current routine counter-productive? I’m just tired of constantly wearing hats and using hair fibers. Any comments/suggestions are appreciated

Another case of topical dutasteride making hair worse. This is mainly why i now stick to only oral dutasteride which is truely proven to work.

How long did you take finasteride and dutasteride respectively? I can imagine your frustration but consider yourself lucky that at your age (34?) you still have a hairline and temple points to frame your face and good coverage in the front. It’s possible all the treatments helped you maintain most of the rest of your hair and prevented much worse crown loss from occurring.

Try to get a prescription for oral minoxidil and also start the process of researching hair transplant surgeons and scheduling consultations. You will want to know how suitable your donor area is and what your options are if that crown loss worsens further.

See your doctor about starting oral minoxidil and oral dutasteride. I'm on oral dutasteride 0.5mg every other day and oral minoxidil 2.5mg daily and I wish I had started using these meds much sooner. A lot of my patients wish they had started these meds much earlier too
I think I will switch to from Topical to Oral Dur/Min. The topical has made my scalp flake and become more sensitive

I’ve been taking topical dutasteride since June 2021, and oral finasteride (1/2 mg daily) for 2 years, and topical minoxidil since 2013

I would think about dropping the topical dutasteride then, but before switching to oral dutasteride, you may aswell try taking the actual 1mg daily dose finasteride for a while to see if it makes a difference.

Just get a transplant
I don’t have $10,000 to splurge. Also I got a consultation once, and they said I was still too young and the crown is the most difficult area to do a transplant and that I might not be satisfied with the results due to limited donor area

How old are you?

What is the condition in your family? Father, grandfather?

Quite important for decision hair transplant.
I am 35. Never met my maternal grandfather, but the uncles on my mothers side still have hair well into their 50s/60s. Same with maternal cousins. My father’s side most uncles still have hair in their 60s and 70s. My paternal grandfather went bald in his 70s. My dad is 68 and still has hair, but he is recently thinning in his crown. He had a full head of thick hair well into his 50s. My oldest brother has a full head of very thick hair. My other brother is thinning like me though

I can't tell with your hair combed forward but do you have some temple recession too? Any photos with hair slicked back wet?

Really ? No one ? How long did you stay on the same treatment without changing anything ? For example, for how long have you been using Minoxidil plus Dutasteride topical ?

Just had genetic bad luck in the hair lottery.

You are very similar to my case.
Everyone has hair in my family and I would be a NW5 at least without finasteride.

With your current therapy you can certainly keep your hair status for many many years.
I have been on finasteride for 5 years. First daily finasteride for 3 years and since 2 years 3x a week.

Of course the effectiveness with 3x a week is not as good as with daily finasteride use.
Ie. I continue to thin out.

To your problem:

You could cover tonsure well with 2500 grafts.
