Burning Sensation on Tip of Penis When Ejaculating

Hey guys,

Been on finasteride for a month at 1mg/day, but my orgasms are slighty unpleasant -- I get a burning sensation at the tip of my penis when I ejaculate. Dropped dose down to .5mg/day, but the symptoms are still present, albeit lessened.

Urologist was puzzled -- my UTI test came back negative. Only thing he said to do was take ibuprofen and lay off caffeine (I drink a lot of caffeine).

I'm wondering, have any of you guys experienced this symptom? Did it go away eventually? Did it respond to changes in dosage? How long should I remain on a specific dose before lowering it? Do you think this side effect will disappear with continued use? I've been on it for seven weeks now.

I really don't want to quit finasteride, but I can't keep having orgasms like this.

13-year finasteride user here and that hasn't been an issue at all for me. I have had that happen when I c*m really hard and especially when I had been drinking that night, but that's not finasteride-related I don't think.

Thank you for the reassurance. Do you think I should try and go back on 1 mg per day to see if the side effects will go away? Or stick with 0.5 mg?

Well, dosage is the trick and it's different for everyone. I was on 1mg ED forever, dropped to 0.5mg ED and started shedding after 3 months but felt great (i.e., mentally, sexually), and now (very recently) I'm on 1mg EOD because there's a hypothesis that 1mg EOD is more effective at stopping hairloss than 0.5mg ED even though they average out to the same amount over time. How is your hairloss on 0.5mg ED?

Do you think I would be able to know after two weeks? Also, how do you check? Do you rub your hair and count the amount of hairs that fall out? Should I be doing this regularly?

Too soon to tell on 1mg EOD vs 0.5mg ED because I just started the regimen this week. Whatever dose you take, I wouldn't recommend taking it ED simply because your DHT levels stay suppressed at a fairly good baseline for 2-3 days after just one dose.

The way I test efficacy is plugging the bath tub, make sure any hairs aren't in the tub, and running water thoroughly over my head for like 30 seconds. Then I look in the residual water and see how many hairs are in there. If it's 10 or less, I believe I'm holding my ground. If you see more than 20, you might be losing ground. My method could be flawed but it seems to work for me.

Thank you. I'm going to start doing that. I think I will switch to 1 mg every other day based on what you have told me.

1mg EOD has become a standard across the industry as a good balance of efficacy vs sides, so you can't go wrong with it. If you're still getting sides, drop to 0.5mg EOD and see how that goes. All the while, testing efficacy using the method I suggested (or another one if you have one).

I'll tell you that when I dropped to 0.5mg ED and felt great, it wasn't hard to notice I was losing ground after 3 months because I'd see hairs on my pillow, hairs coming off my head onto the counter when I'd brush my hair, etc. It was obvious to me.

Thank you and best of luck to you as well. I really don't want to lose any more hair; my forehead looks disproportionately large now. If I had caught it a few months earlier, or started taking it before I noticed any signs, I could still be at NW1. I was complacent and in denial.

finasteride can cause retrograde ejaculation which would cause a burning sensation.

Is this something that can go away on its own, or what I have to cease the medication?
I think you would have to stop the medication for it to reverse.
