It looks like I can get it very cheap at the costco pharmacy, but where do I go to get a prescription? Just make an appointment with a dermatologist? And what do I say to the dermatologist, its not obviously apparent I am losing hair. I am worried he/she will just reject me.

Any doctor will be able to prescribe it. Ask your GP first, they will probably not want to do it, but this is the easiest option. I would look for a hair transplant doctor instead of a dermatologist first, as a hair transplant doctor will be more likely to make the prescription. Yes make an appointment. hair transplant doctors are more liberal in their prescriptions for hairloss

Generally speaking, how long of a wait does it take for an appointment with a derm or hair transplant doctor?

I get my script from my GP. If he said no I would call different derms and ask if they prescribe finasteride for hair loss before even bothering to make an appointment. And finasteride at costco is crazy cheap. I got a 4-month supply for $24.
