In today’s case study, we bring you the complete hair restoration journey of a 50-year-old male patient who was suffering from advanced Grade 5–6 baldness. This transformation was carried out under the expert care of Dr. Kiran Chotaliya, leading hair transplant surgeon at Hairfree Hair Grow Clinic.

Despite having no major health concerns like diabetes, blood pressure, or lifestyle addictions, the patient experienced progressive hair loss over the last 5–6 years, mainly affecting the frontal zone, mid-scalp, and crown (vertex) area. This case showcases how advanced baldness can still be treated effectively using the FUE (Follicular Unit Extraction) technique.

Consultation and Diagnosis at Hairfree Hair Grow Clinic:

The patient consulted Dr. Kiran Chotaliya, where a thorough evaluation was done, including:
  • Scalp analysis using trichoscopy
  • Donor area assessment: Sufficient density and thickness at the back and sides of the scalp
  • Feasibility check for FUE
  • Hairline planning based on age, face shape, and realistic expectations
Since the patient was medically fit, had healthy donor density, and understood the outcomes and limitations, he was considered an ideal candidate for a multi-session FUE hair transplant.

Treatment Plan:

  • Technique Used: I-FUE (Implanter Follicular Unit Extraction)
  • Grafts Extracted: 4,200+ grafts over multiple sessions
  • Coverage Area:
    • Frontal zone
    • Mid-scalp
    • Crown (Vertex)
  • PRP Sessions: 5 PRP therapies spaced at 2-month intervals post-transplant
  • Post-Operative Medications:
    • Minoxidil 5%
    • DHT Blockers
    • Multivitamin Supplements

Step-by-Step Procedure:

1. Hairline Design & Planning:

  • A natural-looking, age-appropriate hairline was designed by Dr. Kiran Chotaliya.
  • Slits were made strategically to ensure natural angle, direction, and density distribution from the front to the crown.

2. Donor Area Evaluation:

  • Hair was thick, healthy, and dark with a good number of multiple follicular units.
  • Trichoscopy confirmed donor sustainability for a large number of grafts.

3. Graft Extraction:

  • Grafts were extracted using a 0.8 mm serrated titanium punch.
  • Time taken: 3 hours per session (multi-session plan)
  • Graft Distribution:
    • 22% single-root
    • 60% double-root
    • 18% triple-root grafts

4. Graft Storage:

  • Extracted grafts were preserved in RL solution at 4°C using a graft chiller machine to maintain maximum survival rate.

5. Slit Making & Implantation:

  • Slits were made using a sapphire blade (1.2 mm–1.4 mm) under magnification.
  • Grafts were implanted using K.E.E.P. implanters, categorized by root count for maximum density effect.

Post-Surgery Hair Growth Timeline:

A) Shedding Phase (0–3 Months):

  • Transplanted hairs began to shed gradually (normal post-transplant shedding).
  • 2 PRP sessions were done to boost root strength and reduce fall from existing areas.

B) Baby Hair Phase (3–5 Months):

  • New hair sprouted slowly in all transplanted areas.
  • Hairline began to become visible; the patient could style short hair.
  • Density at the crown area started showing gradual improvement.

C) Growth Phase (6–8 Months):

  • Over 85–90% of the transplanted hairs were actively growing.
  • PRP at 6 months improved thickness and coverage further.
  • Hair density became significantly visible in the front and mid-scalp areas.

D) Final Results (10–12 Months):

  • By 10–12 months, the patient achieved a full scalp transformation.
  • The hairline was natural-looking and dense.
  • The crown area was filled substantially and matched the density in the front.
  • The patient was confident, happy, and looked 10–15 years younger.

Final Outcome and Review:

The patient reported high satisfaction with the procedure and outcome. He was able to style his hair, attend social functions confidently, and even had multiple haircuts post-transplant. With continued use of minoxidil and occasional PRP, he is expected to maintain the results for the long term.
