How bad does this look? loosing diffuse 200 hairs day

Hi my nam is Erik, 36 years old. Alittle over a year ago I took the drug accutane for a long time problem with folliculitus on the back of my head along with bacne. I only took 20mg for 3 months. 1/3 the dose I was supposed to take. One day I started to notice hair loss from everywhere. Sideburns, top, back, ect. I knew accutane could cause a Telogen Effluvium type symptom. I stopped immediatly. Anyways I have been to 6 derms and no one can help. I can gently brush or pull hair from anywhere on my head. NOT PULLING HARD. I had a biopsy done last march and it said suggestive of androgentic alopecia. I have been losing eyebows, eyelashes, my nose hair, everywhere. I then thought some sort of alopecia araeta. My hair is now very thin everywhere. sides front ect. I lose at least 50 when I shower, and at least 50 when brushing anytime during the day that I brush. I had extremely thick hair before. Every Doctor I went to told me my hair looks fine I am a high shedder? My scalp now burns all the time. Itchy all the time, my eyebrows burn. I now how some sort of past undermy eyebrows and flakey skin on my head. I use nizoral, t gel, tea tree oil, ect. ect. I started on propecia about 6 months ago. I has done nothing exept increase the shedding. My eyebrows grow back in very small and then fall out again. This dead skin has to be the reason. Now my TSH has always been on the high side. So i am on 50mcg of synthroid for the last 6 months also. My tsh is ok. I still lose hair, hundreds. Now I am extremly stressed. My mom has had 2 strokes, Im getting ready to have a baby in the next week, my sister got a divorce and mom and dad adopted 3 boys, and I run a car dealership. I know what u will say, its stress. Well this all started before the things I just told you. I beleive the accutane started this, its proven. Now my endo Doctor told me to stop the propecia. I have quit for 3 weeks and do feel like I have more energy. Buy my scalp is crazy itchy. I know male pattern baldness can cause itchy scalp but this is crazy and nothing helps. And Im losing eyebrows, at least 10 per day minimul. I am not crazy but this sh*t is making me nuts. At here is the kicker, last oct. the cable machine broke at the gyme and put a 9inch scare down the middle of my head! Anyone have any ideas. I never had dandruff or eyebrow loss before. It has to be connected with my hairloss? who knows. sorry for the long post how do you post pics I will be more than happy to show you. Funny thing is most people dont notice the hairloss. When My hair is wet its real bad. I keep a #2 on sides and about a 5 or 6 on top. I have always had a pretty receded hair line since high school. My hair just fall out so easyly and my scalp is flaky as sh*t. First two pics are recent, last pic is in april. My hair is alittle weter in the first two. Can you really tell two much. I do have a big cowlick and my hair grows in a circle patter.

Well maybe you are now I've looked again, I think you need to relax, theres meds out there that can help somebody in your situation.

I liked your first reply but I know Im losing hair, i see it falling. Propecia did not seem to halt the hair fall. It is diffuse I know rogaine would not work cant put that all over my head. And if you mean meds for my head, im ok not crazy or nothing.

come on man. One of the biggest problems has been the eyebrow loss. Getting back to the hair thing instead of the sane part. Can the two be connected. Most of the time I have read it is the thyroid. However I see the dermatitis under my brows especially after the shower. I have tried every cream shampoo, steroid ect. Anyone have any of these type symtoms?

I'd deal with the irritation and skin problems first.

You fix that, maybe your hair will grow back, since it's likely it wasn't male pattern baldness in the first place.

I have such oily skin after the accutane. I do not get acne anymore. However there are plugs of keratin that are comin out the hair follicles on my head and especially the eyebrows. Do you think a low dose 20mgs of accutane would help? I hate take the thing that started this whole mess but 20mgs per weeks would be awful low.

Erik,

Judging from your crown picture your hair looks very good in terms of 'balding' or alopecia , especially for your age. No you aren't an old man, but in terms of male pattern baldness, it generally begins in the early 20's, arguably in the late teens but unnoticeable for most. With this in mind, .. with at least 15 years of DHT involved male pattern baldness of some small level you should show a more defined pattern of hair thinning and even typically absolute sections.. ie: The crown in some cases,.. of near complete thinning/bald spot.

I didn't see a picture of your temples or eyebrows so it is very hard to comment other than to go on what you report.

Your hair loss reported is not typical of male patterned baldness.

This doesn't mean you don't have any reason to justify being freaked out -- when we lose hair and don't know why.. it freaks us out. Further, when multiple doctors have no answers about it we are more frustrated and confused.

I would be more worried about an underlying physiological health problem causing the hair loss than male patterned baldness. No, I don't believe you have anything occurring that is deadly.. just an underlying issue that stands to be corrected.

I recommend that you do *not* take accutane again. Especially if your acne is solved as you report. It very well could be that your body is processing the excessive buildup of vitamin A from the accutane in an unusual manner. Perhaps with time, maybe more than you would like, this will resolve just as it came.

Try to not stress about it as much as you possibly can, that will only make things worse. Think of it rationally and I would encourage you continue seeing doctors at a regular pace in an effort to keep your thyroid regulated and continue monitoring and testing for underlying pathology. I would keep a regular established history and visits with a PCP you trust and branch out to multiple specialists until you find some answers.

Hair typically doesn't just fall out from your head, eyebrows, and other places (in great numbers) daily,.. for no reason. On the other hand,.. our hair can only fall out at such a rate before we have undeniable balding of the area(s). So if your hair is truly falling out at an astronomical rate on your head, then it wouldn't take very long to be quite bald. We go bald in the thinnest areas naturally, first. In other words you should be bald or have quite obvious bald spots on your scalp from your reported hair loss and your eyebrows, for example, should be virtually gone.

None the less,.. in my opinion, the hair loss you have very diffusely experienced is not permanent. I also believe this is likely a "condition" that will resolve itself if given enough time without adding in other variables like accutane and so forth to your body.
