One year on Propecia - Am I a non responder? Advice please.

Hello

I'm 31 years of age, and at around 22 I noticed some thinning of my temples and around the crown of my head that went on for many months. My derm recommended I try Minoxidil before going with Propecia, so I got on minoxidil and actually had very good results for about 6 years or so. My crown filled right back up, and my temple loss halted.

Around July 2008, I noticed increased shedding again in my crown and my temples, so I continued my Minoxidil treatments. During this time I also had increased face and scalp oil/acne. After 8 months of watching the outer part of my temples thin out and a little spot growing larger on the back of my head, I decided to get on Propecia in March 2009.

My results;

March - No change
April- No change until a slight reduction in shedding
Late April - Shedding almost completely stopped
May – Heavy Shed #1. Increased oily skin and scalp. Facial Acne. Major Scalp Acne. Itchy scalp. Hair from all over is sheds. At this point I added topical spironolactone
Late June- Slowdown in shedding
Late July - Almost complete stop in shedding
Late August – Heavy Shed #2 Same as first shed except temple and front hairline is severely affected. Crown and vertex not as much. Pulling a lot of hairs that have color and are thick at one end, but thin and colorless for anywhere from last 20% to 50% of the strand. A lot of short, small, damaged hairs shed here too.
September - Heavy shedding continues - I switch to 1/5 Proscar.
October - Shedding continues
November - Slowdown in shedding - At this point I added Fluridil and Oral spironolactone ( started on 25mg, now on 100mg)
December - Longest slowdown in shedding yet
January - Heavy Shed # 3- Same as others, but crown and vertex is MAJORLY affected this time. Pull many strong thick healthy hairs from the crown. Quite a few hair bulbs are covered in a hard clear substance. I upped Fluridil to twice a day here
February - Heavy shedding continued

So three short breaks in hair loss followed by 3 massive sheds. During the sheds, my hair gets noticeably worse week to week. During the short non shedding times, my hair stays almost the same week to week.

Overall, I haven't maintained, and the rate at which I'm losing my hair has actually increased since starting Propecia. The outer temples are totally gone and the inner part of the temples as well as the whole front hairline took a big hit in the second shed and I really can't get over how much hair from my crown I lost in just 2 weeks in this most recent shed. I've had to change my hairstyle several times to accommodate and now I'm at the point where I'm slapping on tons of concealer now just to go out. I can't believe just one year after starting Propecia just to maintain, or at least slow down my loss, I'm at the point where I'm considering trying partial lace hair pieces for my temples and crown.

I'm still going through the third shed as a write this, so what do I do? I've added Topical spironolactone, Fluridil, and now even Oral spironolactone ( a desperation move), and I continue to lose a bunch of hair in these every three and a half months sheds. I was really hoping either the Fluridil or the Oral spironolactone would help prevent this shed, but no. Am I just a victim of major hyperandrogenicity? Besides the oily skin, my facial hair has increased including strange places like my cheeks and earlobes. My lobido hasn't lowered like many report, but it's actually has increased.

I'm thinking of trying the the low dose accutane thing because my sebum production is crazy, and it's obvious that DHT and androgen fighting isn't enough for me. But I'm also thinking of dropping the Propecia because my hair got so much worse so fast just two months after starting it, not just in count but in quality. My hair is just so wiry and kinked now. Besides the acne, my scalp feels like sh*t. Itchy, hot, even painful at times.

I was all prepared to see Propecia through for the full two years even after the first two sheds, but this past month has been really disheartening, so any advice would be appreciated.

Hello,
Without pictures we can't tell you exactly what's going on. Shedding is 90% of the time psychologic.

I would tell you to stick with the treatment and go in Propecia instead of Proscar. Take the 1mg pill everyday.
Stick with the topical anti-dht and drop the oral spironolactone (it's a lot more dangerous than propecia about side effects, trust me.).

I started on Propecia, and experienced the same exact effects for 6 months. However I have in recent days gone back to Propecia.

The only side effect I've gotten from the oral spironolactone is increased hair loss.

When you start take a medication, you can't stop to use it, otherwise you'll lose the hairs you got. Good luck with your transformation ;)

All I did was switch over from Propecia to Proscar, and now back to Propecia again.

They are both finasteride, and I've had the same effects one both. So I've never actually stopped finasteride, just how I'm getting it.

Hey just thought id add, I am kinda in the same boat as you. Im 26 and 13 months in on propecia, the real deal, and think its getting worse. The front third has thinned out. I havnt had any major shed where i see tons of hairs failling, its just thinner and there are def less hairs. I do know however there are lots of people that get results in year 2, however i think the results arnt spectatcular, just at some point u stop losing what you have and start maintaing. My derm tol me not to worry about it and that its working. (he says it works for every patient he had) I didnt take any pictures so i guess maybe it is working somewhat and the loss is worse in my head. I would say i was a Norwood 2.5 when starting and now im pretty much a 3 in the front, the crown is still ok(I think). Well never know where we would be without the meds, so I think the only option is to keep on trucking, since I know im def screwed without them. Good luck and stay in touch, since were both going through the same sh*t.

AS for the accutane thing, I would stay away from that, as it can cause permenant hairloss as a side effect.

heynow, did you go through shedding periods? Any other effects?

As for the Accutane, it's my understanding that there have been hair loss reports due to Accutane at full dose.

But at once or twice a week it may be enough just to take the edge off the excessive oiliness, inflammation and acne on my scalp I've been experiencing for almost two years to provide a better environment for my AA's and topicals to work.

My skin is insanely oily, and other people with the same problem have done the low dose thing and said it worked for them.

I never had any periods where id see more hair on my pillow or in the shower etc, my hair just contiuns to get worse

Is your hair short or long?

Mine is dark and over 6 inches long, so when I lose when I notice it.

Just another update as another month has past.

It's been almost 3 months since the start of my third major shed and it's still going strong. I just went through another major scalp acne attack over the past 2 weeks and I've lost a ton more hair in that time. My scalp, especially the front and near the part, has been on fire, and I've lost the most hair from there. I'm also experiencing outright pain in my head as well.

I wrote in my last update that this shed was targeting mostly my crown, but this past month has been absolutely devastating to the front of my head. This was the first shed that really hit the area in the front center of my head behind my widow's peak. The temples are worse then ever and the hairline took another major hit. I still have decent coverage, but if this continues I'm heading towards Jude Law territory within a year.

I'm really nervous becuase I realized that I am in fact regrowing the hair around my hairline, but instead of growing 5-6 inches like it did when it fell out, now it's only making it to an inch or so before falling out. That can't be good right?

I'm also seeing more of the hairs that start out normal but as it travels towards the bulb, it gets finer and sometimes goes completely translucent for the last inch or so.

Besides the hair issues, I've been horny as f*ck the past few weeks, and for some reason my breath smells awful during these sheds. WTF is that about?

I've just added small dose accutane (only 1 a week to start) this past week to try to help with the scalp acne, oiliness, and inflammation.

I'm also considering dutasteride.

Another little update.

I woke up yesterday with pimples all over the sides and back of my scalp.

I shampooed, and wound up pulling out 100+ hairs from just the back and sides from my head alone.


I'm f*cked.

Kenneth, I've been loosing hair since I'm 19 or so and I'm 27 now. I loose at vertex and top scalp. I also understand what you mean by "hot scalp" it just feels like its very heated and when you touch it you can feel its hotter than other places. I also suffer from bad acne and VERY oily skin. I got my skin into balance from using the lotion from acne.org as a side note. Anyway, going back to the hair - Throughout the years I've tried many shampoos, but i still feel oily and dirty after using them. Than one day I've became so desperate and brought Dr. Bronners's All One pure-castile soap just to expect another bad one. Surprisingly, it cleans my scalp VERY WELL and it is less inflamed now. I picked the Hemp Tea Tree one since i thought tea goes against oil. Anyway, hope that helps. I use it to wash my face too and it gets rid of oil pretty good. I wash my head 2x with it just to make sure my dirts and grease are gone and it feels pretty good afterward. But dont' expect to cure hair loss since I'm still loosing now after using it. It just makes my scalp feels calmer and cleaner. I'm on 3rd month propecia. So far no result. I'm still loosing 100+- hairs JUST from the top part of my head alone. I know it because I use a bucket to wash my hair so i can count all of them. It seems like on month 2 mark It dropped to 60+- hairs / day but than month 2.5 mark goes back to 100+- hairs / day. I'm hoping this propecia will work for me because I'm very depressed from hair loss. This is really killing me. Hope the pure castile soap works good for you as a shampoo. Tomorrow's my 11 month mark on finasteride. Still no improvement, just getting worse. No major visible shedding, but it's getting thinner and thinner all the time. :( Next 1-2 months will decide whether I continue or quit. At the moment the latter option seems much more probable, unfortunately.

I gave finasteride a solid 8 months to show some sign of regrowth, but unfortunately I did not see it. If I gave it more time it may be possible that it could regrow hair on its own, as some users report this at around the 1.5- 2 year mark.

Hey guys, thanks for the replies. Sorry for not responding sooner, I was really stressed the past few weeks, so I just wanted to take some time off from worrying about this whole thing.

Anyway, the good news is that my scalp acne has subsided after almost a month straight of wave after wave of them. It's amazing how I can go from being completely covered in scalp acne for a month to being completely clear in a matter of days.

The bad news is that I continue to shed heavily. I counted about 75 from sides/back and 100+ from the top of my head in the shower today. I'm shedding the long thick hairs that in the past have been an indicator that a major shedding phase was beginning, so I'm freaked that I'm going through back to back sheds and this one is just starting. It's either that, or I'm a week away from this third shedding period going on for at least 4 months straight. Whatever it is, this shed is by far the longest and has done the most damage. Another one of this caliber will be the death knell for my hairline.

For the first time in a few weeks I pulled my hairline back to take a good look and it's official; I have the receding hairline look. I have a noticeable hole on the right temple where there isn't much hair left, and the rest of the hairline has become sparse and uneven compared to just a few months ago.

Like I said before, I've been shedding almost consistently for a year plus, but most of this hairline damage has just been just the past 2/3 months (month 11-13 on finasteride). The areas where I was experiencing the most heat (I don't describe it as a burning, it's more of a bubbling heat feeling) are the areas where I lost the most hair.

I'm also noticing an increase in the "thick at one end, then gradually thinner until translucent at the root end" hairs. I used to see these hairs once or twice a weak in the shower during shedding periods, but now I see at least 2 of these hairs every day. The other day I saw 5 in one shower. It's disconcerting to say the least.

I don't know... I really think I'm screwed. I'm going to give dutasteride a 6 month shot I suppose. finasteride/spironolactone/Fluridil haven't been enough. I can't believe a year after starting finasteride for some slight but consistent temple and crown shedding that I'm at this point already. No maintaining or slowing down of my loss at all. It only sped up from the 3 month mark on.

After the two antibiotic threads here and at HLH, and my own experience with regrowth while on Cipro/Lavaquin, I'm really tempted to do a round of antibiotics even though I know they're not something to be experimented with. It's just that bad, and I'm that desperate.

Heh, my hair has looked as good as your now last time 9 years ago. Today I don't even dream about reaching that stage anymore.
