Hi all,
I recently ordered some finasteride from QHI because I have heard good things from them. That was 2 and a half weeks ago. I know I have to sign an indemnity form for my first order and was thought that they would send me the form, but they haven't. Over two weeks for the finasteride is ok but i dont know why it would take so long to send a form.
Anyway, getting to the point of this post, I have decided to print the form myself and fax it in. The only problem is that the credit card used is not in my name. Whose signature do i need - mine?/credit card holders?
