The importance of nizoral?

Hey everyone,
Im 21 and been taking .5mg of finasteride everyday for a month now. Had some watery sperm for like a week but it seems to be back to its normal state. So I think I'm on my merry way for the dreaded 12 month haul now. My question is, I have no itchiness/tightness/inflammation/dandruff etc. and because I have been shedding, I really don't fathom the idea of adding a shampoo thats possibly gonna make me shed even more. Some people on here swear by Nizoral and others seem to be fine without it. What do you guys think? Will it be completely detrimental to leave out nizoral when on finasteride, or is it something I can play by ear and maybe add in if I get itchiness/dandruff?

I currently alternate shampooing with Nioxin and some other stuff.

I would argue that at the very minimum you should be using a shampoo that is anti-inflammatory (keep in mind inflammation can occur even if you don't feel it physically). Nizoral is special in the category of shampoos for being both anti-inflammatory AND anti-androgenic, with studies indicating it stimulates growth similar to minoxidil. If Nizoral doesn't sound like your cup of tea you may want to use something like Neutrogena T/Gel, which was what all subjects in the 2-year Propecia study were required to use.

what is the purpose of an anti - inflammatory, does propecia cause inflammation in the hair follicles? Can someone please explain this to me, I don't quite understand how inflammation plays a part in this.

male pattern baldness even in its early stages is accompanied by subclinical inflammation. As it proceeds, follicles reduce in size and healthy skin is replaced by fibrotic tissue. Setting aside that Nizoral by itself has some anti-androgenetic properties, a not inflamed scalp is the best environment for regrowth, in other words it is important to have healthy scalp skin so that your topicals are absorbed well and your treatments reach their full potential.

"Ketoconazole Shampoo: Effect of Long-Term Use in Androgenic Alopecia", Pierard-Franchimont et al, Dermatology 1998;196:474-477.

"...Left untreated, androgen-dependent alopecia progressively deteriorates. This was found in the Androgenetic Alopecia subgroup using the nonmedicated shampoo alone. In contrast, both present studies concur to show an unequivocable effect of a 2% KCZ shampoo on hair variables affected by Androgenetic Alopecia. Hair density and size, and proportion of anagen follicles were all improved. Although the number of subjects was limited in the second study, results obtained compare with minoxidil. It has been stated that medications capable of maintaining the existing hair population should be regarded as effective treatments for Androgenetic Alopecia. The present data suggest that KCZ should enter this group of drugs".

Dihydrotestosterone (DHT) binding to androgen receptors (AR) in hair follicles is commonly accepted as the first step leading to the miniaturizing of follicles associated with androgenetic alopecia (Androgenetic Alopecia). Testosterone is converted to DHT by the enzyme 5alpha-reductase. Finasateride a 5alpha-reducase inhibitor blocks the production of DHT and is currently used to treat Androgenetic Alopecia. The inhibition is not complete but a reduction of DHT systemically and in the scalp is accomplished. Ketoconazole has been clinically shown to be effective in the treatment of Androgenetic Alopecia. In this paper, evidence is presented to support the hypothesis that ketoconazole 2% shampoo has a local disruption of the DHT pathway. It is proposed that using ketoconazole 2% shampoo as an adjunct to finasteride treatment could lead to a more complete inhibition of DHT and thus better treat Androgenetic Alopecia.

Abstract
Ketoconazole, an imidazole anti-fungal agent, has often produced features of androgen deficiency including decreased libido, gynecomastia, impotence, oligospermia, and decreased testosterone levels, in men being treated for chronic mycotic infections. Based on these potent effects on gonadal function in vivo as well as previous work in vitro demonstrating affinity of ketoconazole for receptor proteins for glucocorticoids and 1,25(OH)2 vitamin D3 and for sex steroid binding globulin (SSBG), the binding of ketoconazole to human androgen receptors (AR) in vitro was also examined. Ketoconazole competition with [3H]methyltrienolone (R1881) for androgen binding sites in dispersed, intact cultured human skin fibroblasts was determined at 22 degrees C. Fifty percent displacement of [3H]R1881 binding to AR was achieved by 6.4 +/- 1.8 (SE) x 10(-5) M ketoconazole. Additional binding studies performed with ketoconazole in the presence of increasing amounts of [3H]R1881 showed that the interaction of ketoconazole with AR was competitive when the data were analyzed by the Scatchard method. It should be noted, however, that the dose of ketoconazole required for 50% occupancy of the androgen receptor is not likely to be achieved in vivo, at least in plasma. Finally, androgen binding studies performed with other imidazoles, such as clotrimazole, miconazole, and fluconozole, revealed that in this class of compounds only ketoconazole appears to interact with the androgen receptor. Ketoconazole appears to be the first example of a non-steroidal compound which binds competitively to both SSBG and multiple steroid hormone receptors, suggesting that the ligand binding sites of these proteins share some features in common.

thanks Freak, nice success by the way. I think because I am in the very early stages of balding, I'll skip out on the nizoral until the 12 month mark and then add it in after that, and then maybe add minoxidil the following year. I had bad acne for a while in my teens, and learned that the most frustrating thing is to attack a problem with 10 different meds and topicals and not having a clue whats working and whats not... I don't wanna do that again with my hair and be locked in for life with something that possibly is not working at all.

Whether you use minoxidil or propecia, you should seriously consider Nizoral in your regimen. As SE-Freak pointed out, it puts your scalp in the healthiest state to support the conditions for maintenance/regrowth that other treatments may provide.

I doubt one can go through a shed after starting to use a ketoconazole containing shampoo. Worst can happen, it will dry your scalp skin or even cause some irritation (?) like anything on the market could potentially do. I would use it anyway.

i got me some nizoral yall! i used it for the first time yesterday, left it on for like 3-4 minutes i guess, it didn't seem too bad, my hair didn't get dry and limp like some say, in fact it kind of seemed harder, sort of like beach hair. So now im officially on the Big 2 haha.

How many times a week should nizoral be applied.

I have been applying 2% once per week. I dont like the feeling of my hair afterwards so only use it once a week.

How often is nizoral used in all these studies which show results?

Based on the fact that people get results on nizoral alone leads me to believe that it's pretty important.

I've found if I use it three or four times a week and a good conditioner my hair is fine. If I don't
use the conditioner my hair feels dry and crunchy.

Same as, conditioner is really important...i just mix it in with the nizoral rather than do two separate washes.

From their company:

On the days you are using NIZORAL®, we suggest keeping the conditioner away from the scalp as it may affect the effectiveness of NIZORAL®.

So you may not wanna mix em.

Sounds right to me. You wouldn't use another shampoo after using Nizoral, why would you use a conditioner?

First i've heard of that, cheers bud. Does that mean you should not use conditioner at all, or after using the nizoral?

Probably just after Nizoral, unless you're using another type of shampoo which may have potent antiandrogenic effects.

I use conditioner about once or twice a week, coincident on the same days I use Neutrogena T/Gel.

I guess it means that if you wanna use a conditioner, u should be careful to apply it only to hair and not to scalp....
