What causes testicular ache?

I have had a very difficult time getting a definitive answer on this. From the times when I and most have experienced it - when going up in dosage on finasteride/dutasteride and also in my case when going down I think its safe to say that it has something to do with the feedback effect on the testes of the altered hormonal environment. DHT falls causing the HPTA to call on more testosterone production by the testicles. Or the DHT pathwy is inhibited causing more testosterone to be shunted into estradiol causing the HPTA to try and shut down testosterone production.
Anyone know which one it is? Something else. Any literature search seems to bring up talk of vvariocelles and what have you. Knowing what is happening at this point might give a better clue as to what is going on just before/during the appearance of sides on a drug.
Anyway if you know something, let us know also.

Bro, being monitored by a doctor and being placed on Hormone Replacement Therapy I can say 100% that this is why your nuts hurt. It IS testicular atrophy which is the reason HCG and clomid are used. My Doctor told me also that instead of using HCG throughout your cycle use 1 clomid eod, you don't want to abuse HCG because it wil desensitize your body to LH which you DON'T WANT. LH is what sends the signal for your body to start producing testosterone. When you use synthetic test or any AAS for that matter your pituitary gland receives the signal that there is too much test in the body and stops producing it's own. This is why your nuts atrophy, they simply have no function at that point. Don't take this as a flame, just a little education on how your body works and I'm not pulling this outta my ***, like I said I have a Doctor that monitors me and lets me know what's what as far as the endochrine system works....

So some more evidence (slender and circumstantial though it may be) that feedback from the HPTA (in the case of those on dutasteride/finasteride caused by a rise in estradiol) shutting down testosterone production in the testicles that corresponds to testicular pain. I am gonna look further at this TRT link. If this is true then it should be a common symptom in that community.

When I was applying flutamide my balls hurt like hell. I stoped in 2 days.

We are on the opposite of steriods. We are lowing, not increasing DHT. As stated earlier, our bodies actually increase test production from lack of DHT, were steriod user's bodies lower test production from too much DHT (yeah oversimplified but still you get the point).

Maybe our nuts hurt because they are being worked harder than normal, as our body makes our test production increase from a lack of DHT.

p.s. I recognize the name Woodchuck. Is that from Elitefitness.com? If so...... they don't know sh*t about anything overthere. However, I do enjoy your original question OP, so this isn't a slam on you.

I know that the typical response to finasteride/dutasteride is incrfeased testosterone, decreased DHT and increased estadiol but I am not sure of which of these two situations is happening

1) DHT goes down -> HPTA notices lower DHT and calls for more testosterone -> incerase in testosterone and conversion to estrogen.

2) testosterone conversion to DHT goes way down -> increase in testosterone -> increased aromatisation to estrogen -> HPTA notices high estrogen and responds by decreasing testosterone production.

I believe you are saying its the first one that is going on. I suppose its probably a bit of both, especially during those first few weeks/months when a lot of things are adjusting themselves and a lot of guys who dont have sides later on do experience a drop in libido or whatever.
I would love to find out what exactly corresponds to the testicular pain, nutache, whatever so I could know a little more about what is going on and maybe more about what is causing sides (particularly is it excess estrogen).
As I said it is something that from memory I have noticed when going up or down in dosage on dutasteride/finasteride before so it could well be a symptom of both testicular atrophy/shutdown or growth/overproduction.
