I replied to your same question in another topic in which you asked about it, and here's my answer from there:

I'm not sure either is better out of the day and night S5 cream. From what I can tell, the only difference is that the day cream has caffeine and the night cream has adenosine instead (which promotes sleep).

Thay decided to add adenosine not to promote sleep but to promote hair growth. They are some studies it actually can regrow hair. I'm not sure if it does as I've been using night s5 and it about the same as day s5. but...
I actually will be buying only day s5.

The new formula contains tocopheryl acetate instead of retin-a. Tocopheryl is basically vit e, but it seems to be almost useless in topicals.
However, in the past I tried 2 times the old s5 cream formula, and everytime I had some some scalp irritation and shediing, so I had to discontinue it definitively.
I really wonder if the irritation and the shedding was because of the retin-a and so if the new formula can be less harsh on the scalp.

I think there is little or no difference since the main ingredient is the spironolactone. However, caffeine shold be a mild antinadrogen, whereas adenosine sholud be a growing stimulant similar to minoxidil, but I seriously doubt these 2 substances could have any serious effect on Androgenetic Alopecia. As for the useless tocopheryl acetate, the adding of caffeine and adenosine seems to me just a marketing strategy for the naive customer.
At the moment I am using both of them, the day formula with caffeine in the morning and the noght formula in the evining as suggested.
I used to apply the old formula with retin-a and it always caused me scalp irritation and shedding.
I cannot comment about the efficacy of topical spironolactone, and I hope someone can write here his pesonal feedback about this product.
Finally, I do not think spironolactone cream alone would suffice to block androgen receptors, but may be in combination with ru and/or other topical products it can produce some results.
