But topical testosterone gel gets absorbed systemically? Is the dosing the reason or is there another mechanism between testosterone gel and topical spironolactone cream?

Different vehicles since testosterone gel is made to go systemic, testosterone has a lower molecular weight which might help it absorb easier, spironolactone is a relatively weak antiandrogen so you need high doses of it for antiandrogenic effects, etc

Spironelectone blocks the androgene receptors, lessoning the effects of androgens including testosterone. As far as I know it does get absorbed systemically thought, but I might be wrong.

Exactly. You need 150/200mg of spironolactone for the best antiandrogen effects. If you apply topical what goes systemic is insignificant
