I dont believe it's an insult, when the subject meets the criteria, of the definition.

Someone such as rain man, using it to describe a double board certified oncologist, whom sits on the boards of the 2 most prestigious cancer hospitals in the country... and is a long standing member of Mensa....
in that usage...it's a childish name like
"poo poo head".

You have to consider the source,too, when determining if something is offensive or not....

It's meant as an insult, but no differently than a mouse calling, a cow, a duck. A cow doesn't care...it knows its a cow....and who cares what an insignificant mouse thinks, anyway? :)
